Medical cannabis linked to improved quality of life in UK migraine registry

A new registry study from the United Kingdom suggests that cannabis-based medicinal products may be associated with improved quality of life among patients with migraine. The study used data from the UK Medical Cannabis Registry and included 203 adult migraine patients who were followed for up to 24 months.

Improvements seen across several measures

The researchers analyzed patient-reported outcomes for headache impact, migraine-related disability, anxiety, sleep and overall health-related quality of life. The results showed improvements from baseline in the Headache Impact Test-6, GAD-7, the Single Item Sleep Quality Scale and EQ-5D-5L at all follow-up points up to 24 months. Migraine-related disability, measured with MIDAS, improved up to 12 months.

The study also reported that 48.77 percent of patients achieved a clinically meaningful improvement in HIT-6 after 24 months. For MIDAS, the corresponding figure was 63.05 percent, while 67 percent had a positive improvement in EQ-5D-5L after two years.

THC dose linked to greater chance of improvement

Patients received different types of cannabis-based medicinal products, mainly oils and dried flower for vaporization. At baseline, 71 patients were prescribed oils, 48 patients dried flower and 84 patients a combination of both. During follow-up, the median daily THC dose increased markedly, from 19 mg per day at baseline to 134 mg per day after 24 months.

The researchers found that patients in the highest quartile of daily THC dose had a greater chance of achieving clinically meaningful improvement in MIDAS compared with patients in the lowest quartile. At the same time, the researchers emphasized that the results should be interpreted with caution, partly because the confidence intervals were wide.

Adverse events reported by a smaller group

In total, 31 of 203 patients, or 15.27 percent, reported at least one adverse event. Altogether, 249 adverse events were recorded. The most common were headache, dry mouth, concentration impairment, fatigue, nausea and lethargy. Most adverse events were classified as mild or moderate, but 57 were classified as severe and three as life-threatening or disabling. The three most serious reports involved somnolence, delirium and confusion.

The researchers noted that the adverse events were not assessed for whether they were actually caused by the treatment. This means the study cannot determine which symptoms were directly linked to the cannabis-based products.

The study cannot prove causation

The results are promising, but the study design has several limitations. This was an observational case series without a control group and without blinding. Therefore, it is not possible to determine whether the improvements were caused by the treatment, placebo effects, natural variation in migraine disease or other factors.

The researchers also highlighted selection bias. A large proportion of the patients were already cannabis users before prescription, and men were overrepresented even though migraine is more common among women. In addition, patient-reported data were missing for a substantial proportion of participants at later follow-up points, which may affect the reliability of the results.

Conclusion regarding medical cannabis for migraine

The study shows that cannabis-based medicinal products were associated with improved health-related quality of life, reduced headache impact, better sleep and lower anxiety among migraine patients in a UK registry. The findings strengthen interest in further research, but randomized controlled trials are needed to determine whether the treatment truly causes the observed improvements.
